Connecting Every Person to a Family Doctor or Primary Care Team

Ontario is investing an additional $250 million to connect 600,000 more people to a family doctor or primary care team through the creation or expansion of approximately 78 primary care teams across the province. This investment is part of Ontario’s $3.4 billion Primary Care Action Plan, which is helping bring the province closer to its goal of connecting every Ontarian to primary care by 2029.

Ontario already leads the country, with nearly 90 per cent of residents connected to a primary care provider. Through its plan to add more than 300 new primary care teams and connect two million more people to care, the province is expanding access to high-quality, convenient care closer to home.

By growing the health-care workforce and investing in community-based care, Ontario is ensuring more families can access the care they need, when and where they need it.
